Sports Medicine

All sports injuries must be reported to the Athletic Trainer. The Athletic Trainer will then fill out an Accident Claim form that will be emailed to the parent/guardian. This form serves to reduce or completely eliminate any out of pocket expenses NOT paid by the student's primary insurance. This Claim must be submitted to the insurance company within 90 days of the date of the accident.
