Nurse's Office Services
- The Nurse's Office is open from 7:15 AM - 2:30 PM. Students need a signed passport to enter the Nurse's office.
- All medical and dental notes should be brought to the Nurse's Office to be reviewed by the Nurse. If an absence is indicated, the Nurse will document it as "Medical." The notes are then filed in the student's health record.
- Parents/Guardians will be called if a student is too ill to remain in school. Students will only be dismissed to persons listed on their Emergency Contact List unless parent/guardian indicates so directly to the Nurse.
- If a student is diagnosed with a communicable disease, parents/guardians are asked to inform the Nurses's Office as soon as possible.
Physical Exams and Immunizations
Before enrollment in the Hamden school system, each child must have a physical examination. Additional physical examinations will be required of all students in grades six and ten. Students who do not submit proof of physical examination and required immunizations in accordance with board requirements will be excluded from school. Exemptions from immunizations requirements will be granted only per state statute. Physicals are due by May 1, 2020 to the nurse's office.
Parents and guardians must fill out the form below and bring it to the Nurse's Office.
