PUBLIC NOTICE OF INTENT TO APPLY

21st Century Community Learning Centers (21st CCLC) Grant Application

Monitor My Health, Inc., in partnership with Hamden Public Schools, intends to submit an application to the Connecticut State Department of Education for funding through the 21st Century Community Learning Centers (21st CCLC) Grant Program. The proposed program will provide academic enrichment, youth development, and family engagement opportunities for students attending Church Street Elementary School during both the school year and summer months. A copy of the grant application will be available for public review from July 1, 2026, through July 7, 2026, and may be requested by emailing Katya@monitormyhealth.org. Community members are encouraged to review the application and submit comments during the public review period.
