Covid-19 Daily Self-Assessment 

 

Throughout the school year, we ask parents to use the Covid-19 Daily Self Checklist to monitor your child for symptoms that could be due to Covid-19.  

If any develop, do not send your child to school, contact your child’s health care provider and notify the school nurse. 
 

Continue to have your child wear a mask when outside of your home, wash hands often and model social distancing to keep your child safe from Covid-19.

If your child/family has Possible Covid symptoms or a Positive Covid Test 

If your child has any symptoms of Covid-19, please consult with your healthcare provider.  If your child’s health care professional recommends a Covid-19 test, inform them that Hamden schools require a RT-PCR test (not a rapid test).  
 
Cornell Scott-Hill Health Center is able to provide the test for free; call (203) 503-3000 or see their website at www.cornellscott.org 
 
Please inform the school nurse of when your child will be tested. As soon as you receive the test results, contact the school nurse or principal.  If it is after hours or on the weekend, please email the school principal – klbutler@hamden.org.  
 
Your child cannot return to school without having a doctor’s note with an alternative non-contagious diagnosis or a negative Covid-19 RT-PCR test.

Exposure to someone with Coronavirus

 

  1. Please make sure you follow the following guidelines:
  2. Do not come into the school. NOTIFY the school nurse/employer.
  3. Notify your health care provider for advice.
  4. Quarantine for 14 days. You must quarantine, even if you are tested and receive a negative test
  5. If you get tested. Please advise the school and provide the test results.
  6. If your household members develops symptoms of COVID-19 and is tested, please notify the school.
Continue to practice safety precautions: Wear a mask when outside your home, maintain social distancing, avoid large gatherings, avoid touching your face, and wash your hands frequently.
